DextroMedical is seeking a travel nurse RN Home Health Case Manager, Hospice for a travel nursing job in Colorado Springs, Colorado.

Registered Nurse (RN) – Case Manager (Home Health & Hospice)
Location: Colorado Springs, CO
Shift: Day shift | Monday–Friday | 8:00 AM – 5:00 PM
Contract Length: Contract assignment (no on-call, weekends, or overtime required)

Assignment Details:

Weekly Gross: $2,017.60

Tax-Free Stipend: $1,097.60

Taxable Straight Pay: $920.00

Requirements:

Active Colorado RN license or multistate compact license

Associate’s or Bachelor’s degree in Nursing (or related field)

Reliable transportation and valid car insurance (travel required within Colorado Springs area)

Hospice experience strongly preferred

HCHB (HomeCare HomeBase) experience preferred

Case Management certification preferred

Responsibilities Include:

Perform comprehensive assessments of high-risk members

Collaborate with primary care providers to implement individualized care plans

Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals

Identify and reassess cost-efficient, appropriate levels of care

Arrange non-medical supports (housing, transportation) to ensure compliance with treatment plans

Monitor outcomes: clinical, functional, and financial

Promote health and prevent illness through early intervention

Act as patient advocate while maintaining privacy and confidentiality

Additional Information:

Onsite role with field travel in Colorado Springs area

No weekends, no call, no overtime required
