A managed care organization serving New Hampshire Medicaid members is seeking a Licensed Practical Nurse (LPN) to join its Care Management team in a telephonic role. The LPN will support care coordination, admissions, and member engagement primarily by phone, with occasional in-person visits to skilled nursing facilities (SNFs) or members’ homes as needed (mileage reimbursed). The ideal candidate will have experience in long-term care, post-acute intake, or transitional care coordination and be comfortable working remotely in a structured, fast-paced environment.

You must live within 30 minutes of Manchester.Conduct telephonic outreach to members, families, providers, and facilities to coordinate care, admissions, and services.
Support transitions of care for members entering or discharging from long-term care, SNF, or rehabilitation settings.
Gather and review clinical documentation, assessments, and eligibility information for RN or physician review.
Serve as a liaison between the health plan, facility admissions teams, discharge planners, and providers to ensure timely and efficient placements.
Assist with prior authorization requests, documentation preparation, and Medicaid eligibility verification.
Educate members and families on benefits, services, and expectations during transitions of care.
Document all interactions, activities, and clinical notes accurately in the electronic medical record (EMR) and care management platform.
Escalate complex clinical or social concerns to the RN or Utilization Management team.
Ensure compliance with HIPAA, state regulations, and organizational protocols.
Complete occasional field visits to SNFs or members’ homes in New Hampshire, with mileage reimbursement.
Active, unrestricted LPN license in the State of New Hampshire (required).
Minimum 2 years of nursing experience in long-term care, home health, rehabilitation, admissions, or care coordination.
Familiarity with New Hampshire Medicaid processes, coverage requirements, and facility workflows preferred.
Strong communication, organizational, and documentation skills.
Proficiency in EMR systems and care coordination technology.
Ability to work independently and manage a telephonic caseload in a remote setting.
Experience in managed care, home health, health plan case management, or utilization support.
Background in transitional care, discharge planning, or post-acute intake.
$34/hour with mileage reimbursement for occasional field visits.
