RN Case Manager Hours M-F 8am-5pm Travel to patient homes to do assessments in the Cincinnati - Butler, Clermont, Warren, & Hamilton Co. areas REQUIRED otherwise remote, no office to report to RN Case Manager Compensation The pay for this position is $41.00 an hour Benefits are available to full-time employees A 401(k) with a company match is available for full-time employees. RN Case Manager Responsibilities Utilizes a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual’s benefit plan and/or health needs. Conducts comprehensive assessments of referred member's needs/eligibility. Determines approach to case resolution and/or meeting needs by evaluating member's benefit plan and available internal and external programs/services. Applies and interprets applicable criteria, guidelines, policies, procedures, and regulatory standards while assessing benefits and/or member’s needs. Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. Requires an RN with unrestricted active license.
