Schedule: Monday-Wednesday (AM/PM shifts), with occasional evening and weekend work as needed
Hourly: $21-22 based on experience

Job Overview

The Community Public Health Teams (CPHT) model is a collaborative public health strategy implemented across high-priority micro-service areas in Los Angeles County. Each CPHT is composed of a community-based organization (CBO), a health care partner (HCP), and Department of Public Health (DPH) staff. Together, they form a place-based team focused on improving health equity, reducing disparities, and engaging directly with 8,000–13,000 households through comprehensive outreach and community action.

Essential Duties and Responsibilities:

This is a field-based and outdoor position that requires extensive in-person interaction in various community settings. All duties support the CPHT’s deliverable goals and require the ability to navigate and serve assigned census tracts efficiently. Responsibilities include:

  • Door-to-door canvassing and conducting annual household visits using a buddy system for safety.

  • Performing outdoor community outreach and engagement in residential neighborhoods, community centers, schools, and event locations.

  • Coordinating and participating in on-site health screenings, workshops, and community-based events.

  • Building trust-based relationships with community members and providing face-to-face education and service connections.

  • Navigating and referring community members to appropriate health and social services.

  • Collecting and documenting data through mobile and web-based tools to track community health metrics and outreach progress.

Scope and Travel Requirements

  • This role requires daily travel by foot and by car throughout the Antelope Valley, including neighborhoods such as East Lancaster, Central Lancaster, and surrounding micro-service areas.

  • CHWs must have reliable access to a personal vehicle to drive to and from assigned census tracts, outreach events, meetings, and trainings at locations such as LACDPH offices and Rising Communities.

  • Mileage reimbursement is provided in accordance with AVPH policies.

Deliverable Goals & Performance Metrics

CHWs will contribute directly to the following key outcomes:

  • Perform door-to-door strategies to 11,993 households

  • Conduct and complete Household Assessments across assigned tracts.

  • Refer and connect households to health care and social service systems.

  • Maintain accurate and timely records of outreach activities, referrals, and service delivery.

  • Supporting community convenings and related activities, with outreach as the primary objective.

  • Participate in public health response efforts, such as disease investigation or vaccination outreach.

Experience & Requirements:

  • Hold a High School Diploma preferred but not required.

  • Experience working with underserved communities and familiarity with community resources.

  • Prior experience as a CHW, promotora, or related outreach role is preferred.

  • Proficiency in multiple languages is strongly preferred, though not mandatory.

  • Meet basic health (e.g. TB Test and COVID-19 Vaccination Card) and background check (e.g. Livescan) requirements and physical job standards.

  • Maintain CA driver’s license.

  • Must have access to their own reliable vehicle and maintain active automobile insurance and registration in compliance with California State laws.

  • Ability to be culturally sensitive, tolerant, and non-judgmental.

  • Ability to maintain confidentiality.

  • Proficiency in Word, PowerPoint, Outlook, and Excel.

  • Fluency in both English and Spanish is a plus.

  • Ability to walk 1-3 miles/day (reliable transportation required)

  • Fulfill other program-related duties as assigned.

Physical Requirements and Working Conditions:

  • This is a primarily outdoor and field-based position, requiring regular travel by foot and vehicle within assigned census tracts across the Antelope Valley (e.g., East Lancaster, Central Lancaster).

  • Regular exposure to varied weather conditions.

  • Working with overburdened, often high-risk clients of low economic and varied cultural backgrounds, living in high-risk areas.

  • This position requires lifting/carrying up to 25 lbs., stooping, bending, squatting, sitting on the floor, walking, running (may be needed)

  • While approximately 5% of the role involves in-office responsibilities such as reporting and team meetings, about 95% will consist of field-based work.

  • Must be prepared to navigate high-volume, fast-paced environments and adjust to urgent community health needs with flexibility and professionalism.
